CFAQ Support At Home Program

Our Support at Home program helps seniors stay independent and connected by providing personalised care and practical assistance in the comfort of their own home.

  • Eligibility

    Are You Eligible?

    Our service is designed to assist individuals who are 65 years of age or older (or 50 years of age or older for Australia's Aboriginal and Torres Strait Islander peoples) and experiencing physical limitations that make independent living at home and in the community challenging.

    Our aim is to enhance our clients' quality of life and to delay or prevent the need for long-term residential care by promoting independence. If you find yourself in this situation, we encourage you to apply for our service, as you may be eligible to benefit from our support.

    Funding Classification

    Support at Home Program - Service Information and Price Guide (Effective 1 November 2025).

    Support at Home program funded by the Australian Government and is available to eligible individuals who meet specific assessment criteria. The funding figures are based on the Australian Government Department of Health, Disability and Aged Care publication titled "Aged Care Subsidies and Supplements: Rates of Payments Effective 1 October 2025."

    Funding Made Simple

    Our service charges are funded through government subsidies. To ensure transparency and accountability, we provide our clients with a detailed monthly statement that outlines income and expenditures.

    This statement includes information on the type and amount of subsidies received, the service provided, the package management fee, and the overall balance of unspent funds. By providing this information in an easily understandable format, both service users and their families can quickly assess the financial aspects of our service with ease.

    Apply with Ease

    To apply for our services, you, a family member, friend, or support worker can contact the My Aged Care Centre directly by calling 1800 200 422 or by visiting www.myagedcare.gov.au.
